FOP 89 Active, Retired & Associate Health Care Dental Plan Note If you joined the County Dental Plan, the County does not notify Dominion of your coverage change. Also, if you join Dominion Dental, Dominion will not notify the County. A letter must be submitted during open enrollment to Dominion Dental with your name, social security number, and date stating that you wish to withdraw from the FOP 89 Dominion Dental Plan. If you have information to change or need to add a member of your family, please contact Linda Tran at 703-212-3508. If you would like to cancel your Dominion Dental plan during open enrollment (effective 1/1/2010) please email Linda Tran. You will need to include your name (the member), the last four digits or your social security number, and note that you are terminating your FOP Lodge 89 Dominion Dental plan effective 1/1/2010. *** FOP 89 Associate Member Open Enrollment for the FOP Dental Plan is held annually during the month of November ***
