Insurance.  Doctors that accept the POS plan,
may not accept the Cigna HMO.
 Please call your doctor and be specific as to
which plan you have.

Please note: If you joined the County Dental plan, the County does not notify the FOP or Dominion.  A letter must have been submitted during the open enrollment to Dominion Dental with your name, social security number, and date, stating that you are withdrawing from the FOP #89 Dominion Dental Plan.  If you have any questions, contact Dominion at 1-888-518-5338.  FOP89's Group representative is Wayne Kee.
